ID Theft Affidavit
If you have been wondering how to establish standards or procedures for consumers who wish to file information about identity theft, the government has done the job for you. Now available from the FTC is a model form for an identity theft affidavit. The form is posted at www.consumer.gov/idtheft
One interesting aspect of the identity theft affidavit is the instructions. The FTC advises victims of identity theft to review the disputed accounts with family members or friends who may have information about the accounts or access to them.
