What, if any information return would you complete and submit for reporting the health insurance premiumss you pay for a Director. We complete a 1099 MISC for the board fees we pay this person, but it appears we are not reporting anyplace the value of his health insurance that we pay for him. I would imagine that this is reportable, so is it, and if so, which form is best?

If you are paying for a private plan, I believe that you would just add that amount to the 1099MISC as part of their non-employee compensation. If you are talking about them participating in your bank's qualified health plan, it may be the same, but it also may trigger additional reporting for the health care plan depending on the number of your participants since these are non-employees. I would call your CPA for actual guidance on the information reporting.
