Not that we're had the need for one, but does anyone have a form to use when bank employment is denied due to information contained within a criminal background/investigative report (NOT a credit report).

Examiner states we need one, just in case. Obviously my search capabilites are not grand because I can't find one in any of these forums.

Examiners state things all the time, some are fact and some are fiction. What is their source for needing a form?

Ha..didn't/couldn't cite a source..seems to be a case of "you just should have it, just in case"
