I am pretty sure that I need to use the UCC-1 and UCC-1Ad to complete all of the requirements for filing the "in lieu" financing statement. If our client's name changed since the original filing, do we use her name as it was used for indexing the previous filing in Box 9 or do we put that in Box 10 reflecting the name change? If we put the information in Box 10, do we leave Box 9 blank? Thanks for any information.

The debtor's name in Box 9 should be your debtor's current name as listed on page one of the "in lieu" initial financing statement. I would list in Box 10 your debtor's former name along with the date of your original filing, the filing office where the original was filed and the filing number assigned the original financing statement by the orignal filing office.

The term "in lieu financing statement" comes from 9-706(a) of Revised Article 9. The title of the sub-section is "Initial financing statement in lieu of continuation statement".
