The Standard Flood Hazard Determination Form (SFHDF), which expired on October 31, 2008 has been replaced by FEMA with a December 31, 2011 expiration date. The effective date for mandatory use of the new Form will be June 16, 2009. I can't seem to locate the new form in FEMA's website. Any suggestions?

They will most likely update their site with the new form shortly.
