I know I need to have a report EEO-1 filed on or before September 30, 2003. Anyone know where I can get a blank form? The EEOC website has a sample form, but it has the word sample stamped on it. Thank for your help!

EEOC has simplified the EEO-1 reporting method. Go here EEO-1 Form and you can file on line. You will be assigned a user name and password to use in all future years when filing. Don't forget to have an Affirmative Action Plan on file as well.

I have another question regarding the Headquarters report. Our bank charter is in one branch location, but all of our administration (including CEO, CFO, CCO, etc) reside in another location. Which location is considered to be our headquarters??

Where ever the President/CEO is located is deemed headquarters.
