I've reviewed the applicable sections (as well as the corresponding footnotes) in the BSA exam manual regarding the written BSA/AML compliance program should be board approved and also include the CIP.

But where, if at all, it states the written program should be approved annually? It's understood if there are changes in the program it should be communicated to the board with subsequent approval.

I am not aware of an annual approval requirement.
