We need to do a 3 month review for a new employee. Is there any forms or ideas that anyone uses? We are a small bank and everything is very informal but I would like to get something in place so everyone is on the same page.

IMHO...I would use an abbreviated version of your annual review form. That way the employee can understand a little better what is expected and the supervisor can get a head start on the process. I always used the 3 month review as a chance to make sure that the job is what they were expecting and make sure that it is a right fit. If there are any benefits that kick in at 3 months this is a good time to review those as well.
