IMHO...I would use an abbreviated version of your annual review form. That way the employee can understand a little better what is expected and the supervisor can get a head start on the process. I always used the 3 month review as a chance to make sure that the job is what they were expecting and make sure that it is a right fit. If there are any benefits that kick in at 3 months this is a good time to review those as well.
