Someone from our bank recently attended an ACH seminar and was told that there is a PA law that requires a signature on an Affadavit to be notarized. Also, they said there is a new ACH form without the word affadavit that is required, called a "Written Statement Under Penalties of Perjury?" Is anyone familiar with this or do you know where I can find more information or a copy of the new form?

Yes, we have been using that form. If you send your fax number I will send a copy of our bank's form to you. Always willing to help a fellow banker.
