Customer signed the rescission form, which was completed correctly and includes a date on the form, but the customer did not date next to their signature.

Do we need them to date the form? Or is the fact that our form has a date sufficient proof of when the customer received the ROR disclosures.

There is no signature requirement on a right of rescission form outlined in the model forms except for a place for them to sign if they choose to rescind the transaction.
