Can anyone direct me where I would find the origin of the error and omission form? Legal Counsel has stated that I won't find it in state or federal rules, but possibly secondary market rules. I'm just trying to confirm the wording is correct.

It would imagine it is a secondary market creation. If you are not selling into the secondary market, as far as I am concerned and you should probably ask your attorney about local case law, but I have always treated this document as completely worthless. In what other major contractual obligation can you make the other person sign a form that says if we screwed something up in this contract, you agree that we can just re-do the document or make corrections?
