Our new accounts staff have been using an outdated (2006) matrix that recommends the documentation that should be obtained at account opening for the various types of accounts (e.g., individual, POD, INC, LLC, LLP, trust, etc.). I've tried contacting the vendor to no avail to get a more current version.

Would any of you have such a matrix and be willing to share?
