Our vendor has made some changes to the SAR form specific to Part III. It is my understanding that the institutions information is what is reported in Part III #47-63 and branch activity including anything done at the Main Office is to be recorded in Part III#64-70. Is this correct? Vendor changed it so that if only one branch is involved that branch defaults into Part III #47-63. I am confused.........

If a financial institution recorded in Part III has branches involved in the suspicious activity, record the branch information in Items 64 through 70. Information on up to 99 branches can be recorded in a single SAR. A branch is a location (such as an office or ATM) owned by the financial institution but located separately from the financial institution’s headquarters.
