We have a new employee that stated the bank she was previously employed with added the Appraisal notice to the real estate loan application. They also had the ECOA and Military Status notice on the application. Is anyone else doing this?

Considering that the appraisal notice has to be delivered in a form that the customer can keep and they give you the application back, I can tell you that no compliant institutions use that method.
