We have a customer who held an account in one of our WA State branches, but he was a resident of Arizona when he passed away. We are working with the son to determine which state's affidavit we will need for the Small Estate claim. My thoughts are that since the customer was a resident of Arizona, we will need the affidavit from that state.

Separately, if he uses the AZ affidavit, can the Notary mark out the State and County on the form and change it to the appropriate information without affecting the validity of the affidavit?
