Failure to implement an effective BSA/AML Compliance Program: https://www.fdic.gov/news/news/press/2015/pr15061.html

They knew this was coming and weren't able to right this ship? Citigroup is looking to liquidate the US branch now.

Pulling people out of the ditc...
how does any bank operate today with this type of deficiency? there is no excuse, it is criminal behavior, pure and simple willful neglect.

Are those forms supposed to have signatures? If so why are they not signed?

Often, the copies of orders posted on websites are either unsigned, marked to indicate they were signed (with something like "/S/" or with signatures redacted. I imagine the idea is to prevent the fraudulent use of signatures copied from the internet.

On the other hand, a lot of signatures do appear on websites. CFPB Director Cordray's signature is one example.
