Can you clarify? Are you referring to an internal form that your BSA and/or other departments would use when a case is initiated? (e.g., we have an ISAR)

That is correct. Internal form for case investigations.

The templates for a SAR narrative that I have seen all sucked. It's not a fill in the blank exercise.

Forms used by frontline personnel to push the information upward, oftentimes called "Suspicious Incident Reports," etc. can be very helpful and do lend themselves to a fill in the blank approach.
