The templates for a SAR narrative that I have seen all sucked. It's not a fill in the blank exercise.
Forms used by frontline personnel to push the information upward, oftentimes called "Suspicious Incident Reports," etc. can be very helpful and do lend themselves to a fill in the blank approach.
