We need to revoke an exemption for a previously exempt company. This is one corporation, same tax id number, with two different dba names i.e. Anytown Shell Food Mart and Smalltown Shell Food Mart. Do we need to send in the revocation paperwork for each location or can we send in one indicating the corporate address and the two different dbas?
