The scenario is that we have been informed that the TIN we used on the Designation of Exempt Person for a particular business was correct, but is not the actual name of the business. The name used was actually the d/b/a. Therefore, we were going to amend the Designation, but it is also time to renew it. Can this be done at the same time or should there be two filings? I can't find much guidance on the Designation of Exempt Persons. I think that you are limited to checking one box for Boxes #1 and #2 and so my conclusion is that you cannot amend and renew with the same form. However, I guess I want to bounce it off the rest of you before I do it. Can anyone help me out?

The instructions say:

Items 1 and 2: Check only one of the four boxes in these two items.

With the lack of other guidance, I would say unfortunately, you need two forms.
