By regulation how much information is required to be collected on the beneficiary or intermediary bank. Are we required to have the physical address or just the bank name and routing number.

Piror thread. You don't have to get the information on the beneficiary. However, if you get it, you have to keep it. There is no mention of keeping information on the intermediary bank.
