Are we required in MA to notify either the FDIC or DOB when a SAR is filed?

No, the filing of the SAR itself will be forwarded to the appropriate agency.

The only time you need to notify someone else of the SAR would be for insiders. You must send a copy of the SAR to the Div of Banks and also to FDIC

Absolutely. For the purpose of SAR filing, an insider includes anyone from the board level on down in the organization.
