Interpretation regarding Part III of 104. How has everyone treated this section in regard to "where the transaction takes place"? If you have a number of branches does each one fill in their own address and phone number or in other cases would you merely fill in the central office of the banks address?

I base this on the fact that the BSA officer keeps the central file of logged forms, thus if a question arises wouldn't that be the person first contacted to resolve any questions? Thanks for your time

The instructions for the form say to use the address and phone number of the office where the reported transaction(s) took place; if more than one, use the information for one of the offices where the transactions took place.
