Does anyone have a sample risk analysis for multifactor authentication that you would be willing to e-mail to me?

I have a question about the risk assessment. I know it has to be done now, but will it be something that we have to redo every year or is it like all the other risk assessments that are constantly changing with no written requirement for updating?
