Just got back from a meeting. They wish to have the following:
Savings account at fixed rate for 1 yr. Adjustable each qtr thereafter at a rate determined by the bank. Min deposit $2000.

Is there any disclosure issue that I should be concerned about with this type of product? Other than the norm.

It's a normal T-I-S disclosure with an adjustable rate feature. Be sure to include the same information relative to the new account the same way you stated it. "Fixed for one year" and so on. I really don't think it would be difficult to adapt your current savings disclosure.
