When a customer is requesting a stop payment do we need to give them a copy of the signed form? Also if they request the stop pay over the phone how long do they have to come into the bank to sign the paper?

Under the UCC I believe it is 14 days for a verbal and when followed up by a written statement it is extended 6 mos. I am not sure concerning the signed statement. But probably not required but a good idea to do.
