We are moving towards opening all accounty types (DDA, Sav, CD, etc.) for customers who live out of state and want to mail in there applications. We are planning on adding our Customer information sheet and a check off list on our website. The customer must print out the forms to complete them and return all info by mail. We are not accepting applications online. I can't find anything covering disclosure statements we must make on the website for this. Is there any verbiage that must accompany the forms? I don't believe there is because we are not accepting apps. online. Any comments would be appreciated.

Our website has the Terms of Conditions; Privacy Statement and Online Security Statement all of our web pages within our site (along with the FDIC and Equal Housing etc). The consumers can choose which account they would like to open and click on the link to the application for that account. This page has Instructions on how to open the account and a customer service number along with a link to submit any questions. Once they click on the actual application form to open the account, the application has instructions on how to fill it out and where to mail it. It also has Section 326 Patriot Act Disclosure within the application. After we receive the applications the customer is provided account opening disclosures by mail. We call it a Welcome Kit.
