Since there is no requirement that an ARM Disclosure be signed, what documentation have you done that will satisfy an examiners review that the disclosure was provided to the applicant?

Ours are signed.

We are an OCC regulated bank, and they are satisfied with a copy of the disclosure in the file, along with a cover letter(for mail, internet and telephone applications)that states a list of the required disclosures provided to the applicant. You must list the full name of the disclosure,i.e. 5/1 ARM disclosure not just ARM disclosure. Hope this helps.
