So Cal
Does the requirement for a declarations page or flood insurance application and premium payment as proof of insurance apply only to policies that are under the NFIP or are those two items the only acceptable proof of insurance for policies outside the NFIP as well? Or to put it more simply, would an insurance certificate be acceptable proof for non-NFIP insurance?

I think that would depend on whether or not the agent had the authority to bind actual insurance coverage for the private flood insurance company. Agents do not have the authority to bind coverage for the NFIP program - which drives these provisions in the NFIP rules.
