Question: Where can I find the specific wording for our Patriot Act disclosure?
Answer: The CIP regulation provides the following model language:
IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.
What this means for you:
When you open an account, we will ask for your name,
address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.
BOL has just designed a version of the notice form for you to use. You'll find links to various versions below. To keep the file size low, these are low-res versions. There are high-res, customizable versions for sale in the BOL Banker Store. (See ad below.) The customizable versions can be made to include your institution's logo, the member FDIC sign, and/or the Equal Housing Opportunity logo.
There are two different versions of the low-res sign. One has just the model language from the regulation. The second has slightly altered language that mentions business accounts (version 2). We've made both black and white and color versions of each, as well as full size and half-sized versions.
