Question: I am looking for a comprehensive template for an annual security report to the board of directors. I am the security officer at a credit card bank (there are no loan officers, savings or checking accounts. We only issue the retail and bank credit card of a major retailer.

There is a template for a report to the BOD used by Security Officers at a all-service institution. But it would not be suitable for just a credit card bank. It probably would be easiest to go through your list of requirements and create a very brief, non-detailed report.
