FFIEC Releases Revised Form 004

FFIEC has released a revised form FFIEC 004 for "Report on Indebtedness of Executive Officers and Principal Shareholders and Their Related Interests to Correspondent Banks." The changes are based on suggestions from banks.

The revised form can be used for 1996 reporting or a bank may continue to use the old form. In fact, any form is permissible as long as it includes the information contained on FFIEC Form 004. Reports are due by January 31, 1997 for the calendar year 1996.
