How do you get a DCN number if the acknowledgement is no longer available on the BSA entry site?

I just received two amended CTRs I had filed on 9.11.11 before they changed the rules for amended CTRS. At the time I attached a amended form with just the information that was incorrect on the original CTR. The rule changed on 10.31.11 stating that you made out a complete new CTR indicate it as amends prior report. So now it is 2013 and I have two CTRs to try to efile and can't find any instructions anywhere on how to do this. I tried to file FinCen CTR but it asks for a DOC # 14 numbers long; which of course I do not have. Do you have any idea where to find the documentation to show how to do this?
We have closed visa account files dating back to 1993. Do we have to keep these files or can they be shredded?

A finding in a recent audit may possibly result in a change of procedures for us. I disagree with the finding. Are we indeed required to keep a copy of all Notice of Adverse Action forms when we decline a customer (personal, business) for a checking account? We currently give them a blank form advising them to contact the consumer reporting service (i.e. eFunds) and that is all-we do not even inform them as to why they have been declined. If yes, what information are we required to keep?

What records must a bank keep regarding cashier's checks that are purchased? Do they have to have a log with the number of the check, when and who purchased it, and to whom it was payable? Where must they keep them and how long must they keep carbon copies?
