I am creating a form for our New Account Reps to use to collect information for CIP purposes and then document how the information was verified. We will keep this form for 5 years.

I am asking the customer to indicate their expected weekly cash deposits and if they provide any of the following services:

Money orders
Traveler's checks
Check cashing
Currency dealing or exchange
Stored value
Money transfers

and -

Lotto ticket sales

Anyone have any other ideas?

You may want to check under BSA for some sort of annual review of exemption status form. (New file drawers-can not find mine right now.) Same sort of questions.
