The person benefitting from the transaction(s), in this case the business, would be listed in section A. The person conducting the transaction(s), the woman, would be listed in section B. If there were multiple transactions, do check box C at the top. If you have all of the information for the woman conducting the transaction(s), you do not have to check the box in section B. You would only check a box in section B if that section was incomplete for a specific reason. "Conducted On Own Behalf" would be checked if the same person is both benefitting from the transaction and conducting the transaction. It allows you to not repeat yourself. It doesn't sound like that box is appropriate in your situation.
