Another silly question - a long time ago we were cited for not checking box d "Other" and putting N/A when we were identifying a corporation or non-individual. So today, for some reason, I was reading the instructions and I don't see where it says you have to do that. Am I missing something?

I am with you in that I read somewhere to chedk 'other' and enter N/A and have been doing it that way for some time.

From the CTR instructions:

Item 14. If an Individual, Describe Method Used To Verify Identity. If an individual conducts the transaction(s) on his/her own behalf, his/her identity must be verified by examination of an acceptable document (see General Instructions). For example, check box a if a driver’s license is used to verify an individual’s identity, and enter the state that issued the license and the number in items e and f. If the transaction is conducted by an individual on behalf of another individual not present, or on behalf of an entity, check box “14d” “Other” and enter “NA” on the line provided.

Interesting..............never been cited in an audit or exam for that, and never heard of it at any school or seminar.....I learn something new every day, or at least try to!

It's a useful tidbit to keep in mind when you are completing a CTR in connection with a joint account and you do not have all of the information on the other party to the account.
