Are wires to be stored/retrievable by date AND name? date OR name? Is this a NACHA rule? Could you give me a cite or the correct wording?

For all wire transfers of $3,000 or more, the wires should be retrievable by name. In addition, for transfers executed or received on behalf of established customers from or to an account at the bank, the information must also be retrievable by account number.

This is a Bank Secrecy Act requirement. See 31 CFR 103.33(e) for details.

From a practical standpoint, you will want to be able to retrieve by date too since any request by the government for the information will (hopefully) be limited to a date or date range.
