Under July 2010 requirements Section 226.6(b)(1) it refers to Appendix G - Form G 17. As the format to use for non-HELOC Tabular disclosures at application. We are getting ready to order new application forms but I can't seem to find G 17. Can anyone help me out and provide a link?

The way I read the change as stated in 226.6(b)(1) is that the tabular format must be substantially similar to any of the applicable tables in G-17(FR Vol. 74 January 29, 2009, pages 5431-5434). There are 4 examples (G-17(A) through G-17(D))- I think you can select whichever format best fits your product and go from there.
