I am reviewing all of our forms and I was wondering if any one would be willing to provide a copy of the form they have new employees sign after their initial BSA training?

You can PM it to me if you don't want to post it publicly.

I just have them sign the front page of the powerpoint & enter everyone's date of training on a spreadsheet.

the powerpoint's first slide states:

"The Fine Print

It is the policy of the Bank that all requirements of the Bank Secrecy Act are to be complied with. Willful failure to do so will result in immediate suspension, without pay, or termination.

Employees who observe incidents of suspected violations, or failure to report transactions involving customers or fellow employees, are obligated to report such incidents to Risk Management or the Bank Secrecy Act (BSA) Officer. If the employee does not do so, they could be subject to disciplinary action, up to and including termination, and criminal prosecution.

No employee shall be disciplined for properly reporting suspicious activity."

so everyone who signs - acknowledges our policy. smile

I have a sign in sheet with Name of Training and Date of Training. Each person signs in and then I attach the training materials to it.

I agree with the others (sign-in sheet and copy of materials covered) but make certain your sign-in sheet agrees with all that are required to attend. Examiners/auditors have compared it to our telephone roster and we have had to explain any discrepancies. Missed one who was on vacation during annual reg training (usually have a make-up)and it was mentioned on the exit. Just check and double check.
