we have a business account, an employee attempted to wire herself $1000.00 but was unsuccessfull, thanks to our wire dept.
I only know employee name and social number, no other information on file.
do we file?

Whose employee?

If it was an employee of the business, a SAR is not required, since the activity is below the $5000 threshold.

If it was an employee of the bank, a SAR is mandatory, because the threshold for insiders in $0.
