Does anyone have their customer's sign a "Pickup" log when a customer has had their check order delivered to the bank for pickup? If so, does anyone have a log template?

I don't have a log to send but I think it would be something like:

Date Received/Customer Name/Acct#/#of Boxes/Date picked up/Signature of customer/Employee initials

If you want, you could add a field for ID of the person picking up the checks.

We have a log, its just a spiral notebook that we have customers sign and date. We enter the number of boxes of checks and initial.

We have a safe keeping log which is under dual control and kept in a safe keeping vault. Check orders are to be placed in our dual control safe keeping vault. Customer needs to show ID when picking up, it needs to be recorded, and the customer needs to sign.
