We are not going to be using WBA forms in the future and the forms we will be using do not have a POD claim form. What do others use in stead of this?

Although we use another vendor for the majority of our depository account forms, we have continued to use the WBA Designation of POD Beneficiary and the Claim Form for POD Beneficiary.

Is it safe to assume that your forms vendor has their own Designation of POD Beneficiary form, but not a claim form? If that's the case, I'm not sure what to suggest, other than you may need legal assistance is creating some type of claim form to use.
