Although we use another vendor for the majority of our depository account forms, we have continued to use the WBA Designation of POD Beneficiary and the Claim Form for POD Beneficiary.
Is it safe to assume that your forms vendor has their own Designation of POD Beneficiary form, but not a claim form? If that's the case, I'm not sure what to suggest, other than you may need legal assistance is creating some type of claim form to use.
