Where do I find the HELOC disclosure i am required to give out on HELOCs? I know HUD does the "cost booklet" but i'm looking for most recent version of HELOC disclosure and can't find it?

Look in Reg Z - Appendix G has model clauses...the meat will be found mostly in 226.5, 5b and 6.

Other than maybe a forms vendor, most folks develop their own based on their own criteria.
