This is an odd question. What name goes in the "BSA E-filing Suspicious Activity Report Header" and the title when you e-file?

I typically put the Bank name and date of the SAR filings. It is in my opinion for Bank use for your reference as once the file is submitted it is assigned a specific # from e-filing.

The name you enter here is what will appear in the official filing name/track status section of BSA E-filing. It's the name you give the file when submitting to FinCEN.

For example, you could put "2011-01 ABC Company". This will reference ABC Company had a SAR filed in January 2011.

I use our internal reference number in the heading. That way I can quickly reference which filing goes with the internal file.
