Is the lien status for denied and withdrawn loans always 1?

Not necessarily. If an applicant was explicitly applying for a 2nd lien or for a product (say a home equity loan for example) where your bank only offers it for subordinate liens, then lien status would be "2". ETA: Or, if your bank reports non-dwelling secured HI loans, a denied or withdrawn could be coded as a "3".
