Items 38, 40, 41, and 42. Address. Enter the street
address, city, state, and ZIP Code of the financial
institution where the transaction occurred. If there are
multiple transactions, provide information of the office or
branch where any one of the transactions has occurred.

So let's say, customer A opened the account at the Bank's main branch 1 at 123 Street but the customer conducted the CTR transaction at the branch 2 at 456 Street. Then isn't it correct to enter 456 Street as the address on the CTR? Just want to double check this. Thank you.
