Hello,

I am currently performing an audit of the area that provides various reports to management. The area creates numerous reports that are distributed to various high level management members throughout the bank. Aside from determining if adequate controls are in place to ensure accuracy of the data, I would like to select critical reports to verify actual data on the report is accurate. I would like to send the end users a questionnaire to gauge what they are using the report(s) for in order to determine criticality and aid in my report sample selection. Does anyone have a questionnaire they would be willing to share that may have other questions I should ask the end user, besides the obvious two ("What do you use this report for?" and "Do you make business decisions based on data included in the report?"). Any other thoughts related to this type of audit will be greatly appreciated.
