To meet the "through the initial written communication with a consumer" requirement of the SAFE Act does the NMLS# have to be printed/written on that document or can that document be accompanied by a business card with the number on it?

Where does it say it has to be "on the document"? If you provide a business card (and the card has the number on it) inside the envelope or even staple to the document, you have provided the number with the initial written communication.
