For those willing to share.

Please indicate if your Bank requires only 1 ID or 2 IDs for the purposes of new account opening. Please include your approximate asset size of your bank. If only requiring 1 form of ID, please indicate the primary types used.

If someone says one form of ID ask if they are also running a non-documentary system.

And ID's mean different things to different people. Some banks have primary - i.e., government issued photo ID's and secondary - i.e. SSN card, work ID, school ID, etc. (which are not really IDs at all).

We require one form of primary ID and an additional form of ID as secondary. $350 Mil.

Primary Identification:
-Valid state issued driver’s license
- Valid passport or U.S. Passport ID card
- Valid state issued ID card for example, non-driver photo ID card issued by DMV. (Sheriff’s ID cards are not acceptable).
- U.S. Government identification card (U.S. Armed Forces)

Secondary Identification (Not accepted in lieu of primary identification):
U.S. Government identification card (U.S. Armed Forces)
- Automobile registration
- Credit Card
- Union Card
- Local government ID card
- Original draft registration card.
- Local company identification card (company should be recognizable and reputable)
- Valid Green Card

2 forms--one primary, one secondary. Similar to what jennyfromthebloc said above. We also accept Medicare cards as primary if the customer is elderly.

Point of inquiry - what does your PCI compliance and Information Security program say about recording the complete number and expiration date of a credit card? Has anyone been busted on the PCI side for recording the card number?

We require at least one piece of primary identification which must be an unexpired, government-issued document that includes the client's place of residence or nationality and their photograph. Review of Supplemental Identification Documents may be required to complete ID verification, (Name, Address, Date of Birth, Taxpayer Identification Number).

We also do non-documentary verification.

Primary Identification Documents:
* Driver’s License (Standard Issue or Enhanced Issue) – cannot read “Not Valid for Identification”
* U.S. – States or Territories
* Canadian – Provinces or Territories
* State-issued Identification Card – cannot read “Not Valid for Identification”
* U.S. Military Identification Card
* U.S. Military Dependent Identification Card
* Passport [Any document used must be in English or a language familiar to an Officer and the document must be familiar to bank (or one which can be validated by approved resources).]
* U.S. Passport Card
* Permanent Resident Card
* Resident Alien Card
* Coast Guard-issued Identification Card
* Mexico Matricula Consular ID Card [Current Issue Only]

We require 1 primary ID - everything stated above (except for Matricula) and must be government issued, not expired, etc. We also may require supplementary information for address verification and Tax ID.

In addition we do non-documentary verification.

We are $600MM.

We run a LexisNexis in addition to the id.

For address discrepancies, we will ask for a current utility bill that shows the claimed address.
