Our bank is having problems with employees not following policies and procedures. Employees are not held accountable for violations (so we don’t even want to go there). Where can I find some high profile articles about bank employees being sued for not following the policies and procedures? I have Googled different word groups but can’t seem to find what I am looking for. Just want some concrete cases to drive home the importance of why we have policies and procedures.

I would not worry about procedures since these are created by management and can be changed on a regular basis. Policy violations are a different story since most policies are approved by your board or the appropriate committee of the board. I assume you could have an "off the record" conversation with your internal auditor or regulator.
Not following established procedures can work against a bank in a law suit. Procedures are there for a reason and employees going off on their own can lead to all sorts of problems - just think about customer authentication, not stopping unusual wires or ACH, not checking ID, and on and on.
